Vital Records

The Braintree Clerk's Office may have vital records available for current or former residents who have lived or died while in Braintree or completed their marriage intentions with the Office of the Braintree Town Clerk.

Certified Birth Records

John AdamsThe town will have the birth record of any person whose parents resided in Braintree at the time of the child's birth.  Long Form contains all information at time of birth (parents, place of birth, physician, etc.) The long form is needed for passports, insurance purposes, etc.

Certified Marriage Records

Wedding RingThe town will have the marriage record for any couple who filed their marriage intention (marriage license application) with the Town of Braintree.

Certified Death Records

The town will have the death record for any person who passed away in Braintree or was a resident of Braintree at time of death.



Requesting Vital Records

The Town Clerk's Office has vital records for the Town dating back to 1640 to the present.  You can obtain certified copies of vital records (Birth, Marriage or Death Certificates) online, by mail or in-person.

To order by mail, please fill out the Request for Vital Record form (PDF), and submit with appropriate amount of payment and include a self-addressed stamped envelope for the return of documents. 


Town Clerk Office

You can obtain vital records in-person at the Town Clerk's Office during normal business hours.  Clerk's office is located on the first floor of town hall - side parking lot door.

Certified copies are $10 each and be paid by cash, check or debit/credit card. (A service fee applied to all card transactions)

Note: Some vital records are classified as "restricted" and can only be obtained by the person listed on the document.  It is highly recommended that you have a photo ID to present in such instances.
